Requirements for Age Declaration Document

• Valid Nigerian Passport of the applicant
• Valid Nigerian Passport of the declarant
• Declarant must be six years or older than applicant
• Photocopy of passport data page of declarant and applicant
• Two passport size full face photographs of both applicant and applicant
