Requirements for Renewal/Reissue of e-Passport
- Passport Application form to be completed online
- Payment Slip
- Acknowledgment Slip
- Expired/Expiring Nigerian Passport
- Photocopy of Applicant’s data page of Nigerian Passport
- Two (2) current passport-size photographs
- For applicants between the ages of 0-17, please pay close attention to the next section, as there are additional requirements for minors.

Requirements for New Passport (Adult)
- Completed online application
- Copy of birth certificate
- Payment Slip
- Acknowledgment Slip
- Two (2) current passport-size photographs
- Completed guarantors form printed from portal.immigration.gov.ng
- Copy of Passport data page of Guarantor.
- One passport photograph of Guarantor.

Requirements for Persons under the Age of 18 (Fresh Passport)
Applicants under the age of 18 (eighteen) are required to provide the following:
- Completed online application
- Copy of child’s birth certificate (Translated to English if in Another Language)
- Letter of Consent jointly signed by child’s both parents
- Photocopies of both parents’ passports data page. One of the passport data pages must be a Nigerian Passport.
- Payment Slip
- Acknowledgment Slip
- Child’s current two passport-size photographs

Requirements for Replacement of Lost Passport
- Police Report
- Submission of completed online e-passport application form (The link to the online application is in paragraph 7 above.)
- Two (2) passport-size photographs preferably with white background
- Applicant should provide Passport Number of lost Passport (if applicable)
NOTE: The application will be forwarded to Headquarter for Approval. Applicant will be called to come for capturing when an approval is sent from headquarter.

Passport Amendment (Change of Data)
- Original Birth certificate
- Affidavit for change of Name
- Newspaper publication
- Local Government of Origin certificate
- West Africa Examination (WAEC) certificate (if available)
- Primary School leaving certificate.
- Two Passport Photos
- Current E-Passport (if missing, include a Police Report)
- Marriage certificate (If change due to marriage).

- Appointment days for passport services at the Embassy are on Mondays and Wednesdays; from 10am – 2pm
